Medulous, Inc. takes precautions to protect your information and data. When you submit sensitive information via the Medulous website, your information is protected both online and offline. ?Wherever we collect sensitive information (such as credit card data), that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a closed lock icon at the bottom of your web browser, or looking for "https" at the beginning of the address of the web page. We also protect your information offline. Only employees who need the information to perform a specific job (for example, billing or customer service) are granted access to personally identifiable information. The computers/servers in which we store personally identifiable information are kept in a secure environment.

Medulous, Inc. Privacy Policy

Medulous Inc. (“MEDULOUS” or “we” or “us”) created this privacy policy ("Privacy Policy") in order to demonstrate our commitment to user, visitor, subscriber, and customer privacy with regard to MEDULOUS’s websites, including but not limited to this MEDULOUS web site (the “Website”), and to give you confidence as you use the Website and demonstrate our commitment to fair information practices. Privacy on the Website is of great importance to us. Because we gather important information from our users, visitors and customers (collectively “Users” or “you” or “your”), we have established this Privacy Policy as a means to communicate our information gathering and sharing practices. By accessing the Website, you acknowledge and agree to this Privacy Policy and also agree to the Terms of Use of the Website located at http://www.medulous.com/terms.html. Information We Collect. We require Users who use, register to use and/or purchase from the Website (collectively, the "Service") to provide us with certain information that personally identifies you as described below, which may include, without limitation, the User’s name, telephone number, e-mail address, cookies, IP logs, social media account information (for optional login feature), billing information or credit card number (if applicable), and other information (collectively “Personal Information”).

Personal Information Collected or Required. In order for you to access the Website and our basic service we require you to provide Contact Data such as your name and e-mail address. In addition, for you to access other services or purchase products or services that we may offer via the Website, Personal Information may include the following categories of information, if applicable to the services offered by MEDULOUS and selected by you at your option; Financial Data (such as your account or credit card number); and Demographic Data (such as your zip code, age, and income). If you communicate with us by e-mail, post messages to any of our chat groups, bulletin boards, or forums, or otherwise complete online forms, surveys, or contest entries, any information provided in such communication may be collected as Personal Information. If the User is a health care provider we may require additional Personal Information such as education background, current licenses, National Provider Identification Number, certifications, address, or litigation/legal history. Contact Data, Financial Data and Demographic Data are all included in the definition of Personal Information as used in this Privacy Policy.

Traffic Data. We automatically track and collect the following categories of information when you visit the Website: (1) IP addresses; (2) domain servers; (3) types of computers accessing the Website; and (4) types of web browsers used to access the Website (collectively “Traffic Data”). Traffic Data is anonymous information that does not personally identify you but is helpful for marketing purposes or for improving your experience on the Website. Traffic Data is not included in the definition of Personal Information as used in this Privacy Policy.

Use of Information. We use Contact Data to send you information about our company or our products or services, or promotional material from some of our partners, or to contact you when necessary. We use Financial Data to verify your qualifications for certain products or services and to bill you for products and services. You will have an opportunity to unsubscribe to any emails or mailings by clicking on an "unsubscribe" hyperlink contained in promotional emails we send you, or by contacting us by email at info@medulous.com. Even if you are removed from any such list, if you use the Service, you will continue to receive email correspondence from MEDULOUS related to the Service.

Cookies. When you view the Website, we may store some information on your computer's hard drive. This information will be in the form of a "Cookie." Cookies enable us to (i) customize content specific to your interests, (ii) ensure that you do not see the same advertisement repeatedly, (iii) store your password so you do not have to re-enter it each time you visit the Website, or (iv) establish a shopping cart session to allow you to purchase products or services from the Website. Most web browsers automatically accept Cookies, but you can change your browser to prevent that. Without accepting a Cookie, your use of the Website may be affected.

Sharing of Personal Information. We share certain categories of information we collect from you in the ways described in this Privacy Policy. We share Demographic Data with advertisers and other third parties only on an aggregate (i.e., non-personally-identifiable) basis. We share Contact Data with other companies who may want to send you information about their products or services, unless you have specifically requested that we not share Contact Data with such companies. Except as we explicitly state at the time we request information, or as provided for herein or in the MEDULOUS Terms of Use Agreement, we do not disclose to third parties the Financial Data provided by Users, other than our Partners as defined above or to third-party intermediaries to manage credit card processing and we may share any appropriate information with them. All Financial Data that we collect through the Website is used solely in connection with the Service and is not used by MEDULOUS for marketing or promotional purposes, except as provided for herein.

Opt-Out Options. Except for Contact Data which is required to use the Website, you may choose not to provide us with any other Personal Information, and you will still be able to access and use much of the Website; however you will not be able to access and use those portions of the Website that require your Personal Information. If you do not want us to share your Personal Information with any third parties or if you do not wish to receive information and promotional material from us or our partners, email us at webadmin@medulous.com, but please understand that such a request to “opt out” will likely limit your ability to take advantage of all of the features and services we offer on the Website. In addition, we maintain a procedure for you to review and request changes to your Personal Information, as described below. Any opt-out request by you is not deemed valid until processed by MEDULOUS. It is your obligation to verify that you have been opted-out. MEDULOUS shall not be liable for problems with the opt-out procedures.

Changes to Personal Information. To confirm that your Personal Information is correct and current, and to update or remove your Personal Information, you can log into your MEDULOUS account to access your personal profile. Through your personal profile you may: (a) review and update your Personal Information that we have already collected; (b) choose whether or not you wish us to send you information about our company, or promotional material from some of our partners; and/or (c) choose whether or not you wish for us to share your Personal Information with third parties. You are advised that archival or back-up copies of said information will not cease to exist and MEDULOUS shall not be liable for any problems in this editing process. You can view your MEDULOUS account to confirm that your edits have been made to the extent available on the Website.

Confidentiality and Security of Personal Information. Except as otherwise provided in this Privacy Policy, we will keep your Personal Information private and will not share it with third parties, unless such disclosure is necessary to: (a) comply with a court order or other legal process; (b) protect our rights or property; or (c) enforce our Terms of Use. Your Personal Information is stored on secure servers that are not accessible by third parties other than our Partners. We provide you with the capability to transmit your Personal Information via secured and encrypted channels if you use a similarly equipped web browser.

Additional Information. In addition to Personal Information and Traffic Data recited above, we may also request or collect other information regarding your use of the Website, which if applicable, will also be expressly included herein as Personal Information. You can opt out of providing this additional information by not entering it or not using the Website, although not providing it may hinder your ability to use the Website and/or the Service.

HIPAA, HITECH and Protected Health Information. The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) as modified and amended by the Health Information Technology for Economic and Clinical Health (“HITECH”) Act of 2009 contains provisions regarding the use and disclosure of Protected Health Information (“PHI”). PHI includes any information about health status, provision of health care, or payment for health care that can be linked to a specific individual and any part of a patient’s medical record or payment history. Users are prohibited from submitting or posting PHI to the Website and MEDULOUS hereby expressly disclaims any responsibility for any such action by any User. Lost or Stolen Information. You must promptly notify us if your credit card, user name, or password is lost, stolen, or used without permission. In such an event, we will remove that credit card number, user name, or password from your MEDULOUS account and update our records accordingly.

Third Party Websites. The Website may contain links to other websites not part of MEDULOUS. We are not responsible for the privacy practices or the content of these other websites. You must check the privacy policy statements of other websites to understand their policies. When you access a linked website you may be disclosing your private information. It is your responsibility to keep such information private and confidential. We have no control over its use on linked websites and you must exercise caution when deciding to disclose your Personal Information. We also make chat rooms, forums, message boards, and news groups available to you. You are advised and understand that any information that is disclosed in these areas becomes public information.

Security. You are advised that any information or data that is transported over an open network, such as the Internet or e-mail, may be accessible to anybody. We cannot guarantee the confidentiality of any communication or material transmitted via such open networks. When disclosing any personal information via an open network, you should be aware that it is potentially accessible to others, and consequently, can be collected and used by others without your consent.
